Anesthesia and Positioning

Tamponade and Wound Closure

Fluid-air exchange replaces PFCL if used; tamponade agent is then injected (air, SF6, C3F8 gas, or silicone oil depending on diagnosis and retinal status). Trocar cannulas are removed; 25G/27G wounds typically self-seal without sutures. Subconjunctival antibiotic and steroid injection, antibiotic ointment, and eye patch are applied.

Postoperative Positioning Instruction

Macular hole patients immediately begin face-down positioning. Retinal detachment patients assume the position that keeps the gas bubble in contact with the treated break. Nurses provide detailed, confirmed instruction before the patient leaves the operating suite.

PPV is a single procedure, though secondary surgery may be needed (silicone oil removal at 3-6 months; repeat PPV for complex retinal re-detachment). Postoperative antibiotic, steroid, and mydriatic drops are required for 4-6 weeks.
