If periodontal pockets, attachment loss, or subgingival calculus are present, supragingival scaling alone is not enough to control periodontitis, and further periodontal examination plus subgingival scaling/root planing are needed.

Intraoral disinfection is performed. The patient rinses with an appropriate mouthwash as instructed, such as chlorhexidine, to reduce aerosol contamination.
Scaling instruments are used to remove plaque and calculus.
For heavily stained tooth surfaces, air polishing is used to remove stains and plaque.
A rubber cup or polishing brush with polishing paste is used to smooth tooth surfaces, especially interdental spaces and stained areas, reducing surface roughness.
Tooth surfaces are rinsed with saline. Medication such as iodine glycerin may be applied to bleeding areas when needed.
Usually completed in one visit. If calculus is heavy, gingival inflammation is obvious, or air polishing is needed, treatment time may be longer or divided into sessions.
