A treatment that combines intravenous photosensitizer injection with specific-wavelength laser activation to selectively close abnormal neovascularization.

Photodynamic therapy (PDT) involves intravenous injection of the photosensitizer verteporfin (Visudyne), which accumulates in choroidal neovascularization. A 689nm non-thermal laser then activates the drug in the lesion area, generating reactive oxygen species that selectively damage neovascular endothelial cells, causing vessel closure. PDT minimally affects surrounding normal tissue and is suitable for specific AMD subtypes (polypoidal choroidal vasculopathy/PCV) and certain macular neovascularization. Strict light avoidance for 48 hours post-treatment is required.

IV access establishment

Establish IV access; slowly infuse verteporfin (6mg/m² BSA) over 10 minutes.

Drug distribution period

Wait 5 minutes after infusion for drug concentration in neovascularization.

Laser irradiation

Apply 689nm laser to the lesion area through slit-lamp and contact lens for 83 seconds (spot size covering lesion extent).

Light avoidance instructions

Instruct patient on strict 48-hour avoidance of sunlight and bright light, including skin exposure.

Evaluation typically every 3 months; repeat treatment based on angiography and OCT findings. Often combined with anti-VEGF injections.
