Chalazion excision is the standard treatment for chalazia unresponsive to conservative management. Surgery is typically performed from the conjunctival surface (inner eyelid), curetteing granulomatous tissue and the cyst wall without any external skin incision or scarring. Performed under local anesthesia in approximately 10-20 minutes. Children may require topical or brief general anesthesia. Recovery is rapid with normal activities in 1-3 days. For elderly patients with recurrence at the same site, excised tissue should be sent for pathology to rule out meibomian gland carcinoma.

Disinfect eyelid skin and conjunctiva; subconjunctival local infiltration anesthesia.
Secure and evert the eyelid with a chalazion clamp, exposing the conjunctival surface lump.
Vertical incision through the conjunctival surface over the chalazion.
Thorough curettage of granulomatous tissue and cyst wall with a curette.
Apply pressure for hemostasis, antibiotic ointment, and pressure dressing.
